◦ Most HMO plans require a referral from your Primary Care Physician (PCP) and an approved authorization before an appointment can be scheduled.
◦ If you schedule without authorization, we will likely need to reschedule or even cancel your appointment.
• Retina, oculoplastic & cosmetic appointments
• Eyeglasses or contact lenses — We do not schedule appointments or provide examinations for glasses or contacts.
Please schedule with the physician listed on your required referral to ensure the most seamless care experience.
